Mounjaro Face: Why the Face Looks Saggy After Weight Loss

The weight came off with Mounjaro, and now the face looks more tired, the jawline reads less defined, or the cheeks appear hollow. Concerns like this are increasingly common — and the changes have acquired names: Mounjaro face, Ozempic face.
What matters is this: it is not that a particular drug damages facial skin directly, but that rapid weight loss outpaces the facial fat layer and skin elasticity, which makes the change more visible. So before starting the medication, it is worth settling how you will read your facial line and in what order to weigh liposuction or lifting.
Why does it happen?
The face shows weight change faster than most people expect. There is a fat layer across the cheeks, the area in front of the cheekbones, the jawline, and around the double chin, and that fat also supports the contour. When weight loss takes it, the face may feel smaller — but hollowing and laxity are often what you notice first.
Volume loss, skin laxity, and changes in creasing after GLP-1 medications are explained by exactly this: rapid weight loss and reduced facial fat. Where skin elasticity has already declined, the skin cannot keep pace with the fat reducing, and jawline laxity and nasolabial folds read more sharply.
We hear versions of this in consultation constantly. The weight came off but the face looks longer. The double chin stayed and the cheeks hollowed. I only want the jawline resolved but I am afraid removing more will leave a dent. So this is not a single problem of “losing facial fat” — it is a question of which fat went and which fat stayed.

Why to check the fat layer before you start
Starting Mounjaro or Wegovy will bring the weight down to some degree. But you do not get to choose the order in which it comes off. You may want it from the abdomen and find the face reduces first, or find the face hollowed while fat under the chin stays exactly where it was.
Deciding at that point to lose more, or to fix it later with lifting, narrows your options rather than widening them. Remove fat from a face that has already lost volume and you get hollowing. Leave the fat under the chin and the jawline stays blurred no matter how much weight comes off.
So before starting, assess the face in parts rather than as one mass. Is the double chin and jawline fat that needs addressing? Are the front of the cheek and the temple volume that needs keeping? Is the deep buccal fat something to reduce, or is what you see actually descent? The same logic is why how to get rid of a double chin separates the fat type from the muscle type.
Sometimes suction comes before lifting
Ultrasound-based lifting delivers thermal energy beneath the skin to prompt a regenerative response. It helps where laxity is mild or reduced elasticity is the main issue — but where there is a genuine fat layer under the chin or across the lower face, lifting alone will not resolve the line.
Facial liposuction addresses the fat layer directly. The cannula passing through the tissue as fat is reduced can bring a tissue response and some skin retraction during healing. But that does not make it “stronger than lifting” or a solution for laxity. Skin elasticity, fat layer thickness, age, and previous procedures create wide differences in outcome.
| Your current state | What to look at first | Why |
|---|---|---|
| Clear double chin and fat under the jaw | Facial liposuction | The jawline can stay heavy even after weight loss |
| Cheeks already hollow, face reads long | Preserving volume, lifting, whether to restore | Removing more deepens hollowing and fatigue |
| Weight loss still moving quickly | Adjusting the timing | Further loss will change the facial line again |
In short, this is a question of sequence. If a remaining fat layer is what makes the line heavy, liposuction can be considered first. If there is little fat and the skin is simply loose or volume has gone, then lifting, restoring volume, and letting weight settle come before suction.
If you are worried, start from where you are now
Being worried about it does not mean everyone should have liposuction first. But if what concerns you is the face looking more sagged after losing weight, establish your current fat layer and elasticity before you start the medication.
At Line Culture Clinic we do not read the face as one mass. The double chin and jawline, the deep buccal fat, the front of the cheek, the temple — we separate what should be reduced from what should be kept. Ultrasound and photographic review establish the fat layers, muscle, laxity, and asymmetry first, and if a GLP-1 medication is part of your plan, the rate of loss and your target weight are factored into the discussion.
Results, recovery time, bruising and swelling, and the degree of skin retraction all vary from person to person. If you are taking a GLP-1 medication or plan to, tell us where you are with it, how fast the weight is coming off, your target, and any previous procedures. Those are what the sequence gets decided from.
